Information & Advice
Our Information & Advice Service is available to people aged 50+ in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend by telephone, plus a face to face drop in and appointment service at our Pontypridd office
Community Outreach Services
Our Community Outreach Team supports older people whose physical condition or functional mental health has deteriorated following a fall or bereavement or major life changing event resulting in isolation. We help them cope with their condition at home and reduce the risk of them being admitted, often unnecessarily, to hospital or prematurely entering residential care.
Our Hospital Discharge Service can offer a life-line to people who need that extra bit of support and encouragement following a hospital stay. For many older people, bereavement or illness, even a fall, can result in depression, a loss of confidence and independence.
Independent Advocacy Service
If you feel that your rights and wishes are not being listened to or if you are concerned that an older person is being financially, physically, psychologically or sexually abused please get in touch, we can help make it stop. Hospital to Home Services
Our Hospital Discharge Service provides 6-8 weeks support to people aged 50+ who have recently been discharged from hospital. This service is available to people living in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend areas.
Many older people live alone or with an elderly partner or relative. After a stay in hospital the transition from hospital ward to home environment can be traumatic. Additional support may be needed during this period of readjustment. Our hospital discharge service complements the services provided by other agencies and is able to liaise on behalf of the service user with other service providers.
Nail Cutting Service
Our Nail Cutting Service is a revelation for hundreds of older people each year. For many older people, arthritic joints and tough overgrown toenails make wearing shoes and walking comfortably almost impossible
This is not a free service. We currently charge £13 for toenails at a clinic or £20, plus mileage from the nearest clinic, for a home visit
